Our Direct Primary Care (DPC) membership gives you unlimited access to your provider, same-day or next-day appointments, Telehealth visits, and discounted labs — all for a simple monthly fee.

Ear Lavage, Laceration Repair and I&D

Men > 20 Plan Includes:
Yearly PSA and Testosterone Lab Screening

Membership Pricing by Age Group

Children (0–17 years): $25/month (with enrolled parent) or $50/month (without parent)
Young Adults (18–29 years): $65/month
Adults (30–49 years): $85/month
Adults (50–64 years): $100/month
Adults 65+: $120/month
Families: $225/month (up to 2 adults + 2 children; $20 each additional child)
One-Time Enrollment Fee: $100 per individual / $200 per family

Membership Fees
Monthly membership fees are based on age group and family plan selection (see attached pricing).
A one-time enrollment fee of $100 per individual or $200 per family applies upon signing.
Payments are due monthly by credit card, debit card, or ACH transfer.
Billing
Charges are billed on the day of enrollment, and on the 1st day of each month prior to services being rendered
with a 12 month contract
Cancellation Policy
Membership may be canceled at any time with a 30-day written notice.
If membership is canceled, the patient remains financially responsible for any outstanding balances.
No refunds will be issued for partial months.
Insurance
We cannot accept HSA or use any insurance for Direct Primary Care
Re-Enrollment
$150 per individual or $300 per family re-enrollment fee will be charged
